Supported Living - Outreach & Cluster Living

We work with Individuals to build networks of connected relationships within their neighbourhoods and greater community. Each Individual has their own Support Team made of people/service providers selected by the Individual including representatives, health care nurses, physiotherapists, family, friends, and any other supports of their choosing.

With Outreach Support, we visit homes to provide support with daily living during the week, either one-on-one or in a group. With Cluster Living, our volunteers support Individuals, or groups of others, who live in close proximity of each other.

These networks may be expanded through attending events, activities, recreation, social events, and/or through employment opportunities that match interests of individuals.
